Soil Log Acceptance Form

Due to the work needed to be done, Atlas Septic cannot and will not be responsible for any damage occurring to the lawn, landscaping, shrubs, asphalt, and sprinkler systems. Atlas Septic will try to avoid any damage from occurring. Atlas Septic will be calling in for utility mark-outs prior to our arrival. Atlas Septic will not be responsible for any miss marked utility lines. 

This is authorization to proceed with performing a soil log. I have read, and agree to, and have received a copy of this soil log authorization.
